What's model of this headset?

go to start and select "PHONE" from the drop down menu, dial #83587 you will see the message "SERVICE CODE"

Enter the 6 digit Activation Code and click OK buttom on the screen, you will see the message "service mode ESN" and the ESN Value.

Push the blue OK button (located beneath the screen) you will see the message "service AKEY- input AKEY" press the blue OK button (you will press the button blue OK for all options).

@tony$$$ do you have the 6 digit activation code? I'm on a Canadian carrier called Telus and the verizon code of '000000' won't work for me, any suggestions? Thanks in Advance...
